The Practical Nurse is assigned to the Patient Aligned Care Team (PACT) as a member of the Primary Care Nursing Operations healthcare team and has the ability to demonstrate the knowledge and ability to provide a full range of practical nursing concepts to patients with a variety of physical and/or behavioral problems.

Duties

The duties of the Practical Nurse assigned to the Patient Aligned Care Team (PACT) in Nursing Operations with the Tulsa CBOC include, but are not limited to: Administers medications accurately and promptly under the general supervision of a Registered Nurse. Demonstrates understanding of the effects and/or reactions caused by medications, treatments, and procedures; also records and reports these observations. Performs personal care, diagnostic support duties and treatments according to procedure under the direction of the professional nurse in a timely manner. Completes assignments in a timely manner with constructive utilization of remaining time. Serves as a member of the interdisciplinary team, interacting with patients, families, professionals, and supportive personnel; organizes patient care assignments to meet the needs of the patients. Clearly demonstrates the ability to comprehend, interpret, and function within the individual's discipline as a licensed practical/vocational nurse. Participates in ongoing activities, education and certification opportunities demonstrating personal accountability for owning one's practice. Identifies changes in patient's physical and emotional condition and reports these changes in a timely manner and initiate appropriate action. Participates in Primary Care Team meetings. Instructs patients and family members on the principles of health, self-care, and protocols for treatments and medications. Utilizes teaching materials to promote self-reliance and independence including methods to prevent complications and facilitate recovery; reinforces instruction provided by other health care professionals. Completes required patient care documentation and records significant observations and actions utilizing the nursing process in a clear and concise manner. Maintains a safe, clean, therapeutic environment and adheres to infection control guidelines. Prevents injury to self and others by complying with established safety procedures and uses proper body mechanics and transfer techniques. Participates in accomplishing unit maintenance tasks which includes checking equipment to ensure that is functioning safely. Practices proper technique during sterile procedures. Demonstrates safe work habits, identifies, and immediately reports any unsafe conditions to the Registered Nurse. Identifies any life-threatening environmental hazards immediately, takes appropriate action, and reports findings to the Registered Nurse, Identifies potential risk and takes necessary precautions to ensure the safety of the patient. Uses proper equipment for all appropriate activities. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Per VA Handbook 5005, Part II, Chapter 3, Section A, Paragraph 3j: No person will be appointed under authority of 38 U.S.C., chapter 73 or 74, to serve in a direct patient-care capacity in VHA who is not proficient in written and spoken English. Licensure: Full, active, current and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia. An LPN/LVN who has or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in chapter 3, section B, paragraph 16 of this part. Education: Graduate of a school of practical or vocational nursing approved by the appropriate State/agency and/or accredited by the National League for Nursing Accrediting Commission NLNAC at the time the program was completed by the applicant. Verification can be obtained from the State approving agency or the National League for Nursing Accrediting Commission. Exceptions in education include the following: (1) Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment. (2) VHA employees who were converted to appointments under 38 U.S.C. 7401(3) in implementation of Public Law 98-160, enacted November 21, 1983, and for VHA employees converted to appointments under 38 U.S.C. 7405 in implementation of Public Law 99-576, enacted October 28, 1986; and for their future personnel actions under this authority. The waiver will not apply to persons appointed under 38 U.S.C. 7401(3) or 7405 after these conversions. (3) Individuals granted a license by a jurisdiction that does not require graduation from an approved school. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade. Practical Nurse GS-3 Qualifications: None beyond the basic requirements. LPNs at this grade level are expected to perform all duties within the scope of their license (e.g., medication administration); however, completed work may require routine or detailed higher level review depending upon the complexity of the duties involved. Practical Nurse GS-4 Qualifications: Employees may be appointed at or advanced to GS-4 if they meet any of the criteria in paragraphs 3b(2)(a) through (c) below: (a) Six months of qualifying experience as an LPN or LVN; OR (b) Graduation from an approved school (which may be waived as provided in paragraph 4 of Section C, and one year of experience that involved nursing care work in a hospital, outpatient clinic ,nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures; OR (c) Graduation from an approved school of at least 24 months duration; OR (d) LPNs at the GS-4 grade level perform all duties expected of employees at the GS-3 grade level; however, they are expected to exercise greater judgment, require less supervision, and to operate in accordance with all established policies, procedure, and techniques. Practical Nurse GS-5 Qualifications: Completion of at least one (1) year of qualifying experience at the GS-4 level or equivalent. Applicant must also demonstrate the required knowledge, skills and abilities (KSAs) as outlined in the assessment questionnaire. Practical Nurse GS-6 Qualifications: Completion of at least one (1) year of additional qualifying experience at the GS-5 level or equivalent, fully meeting all performance requirements for the GS-5 LPN/LVN. Applicant must also demonstrate the required knowledge, skills, and abilities (KSAs) as outlined in the assessment questionnaire. Preferred Experience: Prior Acute Care experience References: VA Handbook 5005/3, Part II, Appendix G13 The full performance level of this vacancy is GS-06. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-03 to GS-06. Physical Requirements: Requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity; must be mature, flexible, sensible and capable of working effectively in stressful situations, able to shift priorities based on patient needs; requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing; transferring patients and objects may be required; must be able to express or exchange ideas by means of spoken and written communication; must be able to view and read information from computer screens.
