This position is located in Pharmacy Service, Memphis VA Medical Center, Memphis,TN. This position provides support to Inpatient, Outpatient and Telephone Care Pharmacy. The Clinical Staff Pharmacist serves a multi-functional role and is specially prepared by pharmacy education and experience to have an advanced knowledge of pharmacology, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics.

Duties

Fills prescriptions by selecting the correct drug and dosage form, reducing the drug from bulk to the quantity prescribed, compounding when necessary, then packaging and labeling the drug in an appropriate manner. Reviews the patient medication profile prior to dispensing drugs and medications. Counsels patients and family members concerning medication directions for use, side effects, precautions, and the need to strictly follow the regimen. Monitors patient medication profiles and ensures information is correct and up tp date. Provides consultation to physicians, nurses, and other health professionals on medication management issues. Makes recommendations in the selection of formulary alternative and/or therapeutic interchange medications when those requested are not available or when a more clinically effective or economical approach appears appropriate to patient needs. Serves as a consultant for proper parental medication therapy and scientific compounding techniques for physicians, nurses, and other members of the health care team. Conducts drug usage evaluations, and patient drug therapy monitoring in accordance with VAMC Memphis and Pharmacy Service policy and Joint Commission standards. Applies pharmacokinetics, using rate equations as appropriate to equilibrate serum levels using volumes of distribution and/or renal and hepatic clearances when necessary. Analyzes laboratory test data so as to recommend drug dosing adjustments when necessary and minimize interactions. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. Grade Determinations: GS12: 1 year of experience equivalent to the next lower grade level, or Completion of an ACPE-accredited Pharm.D. program. A pharmacist in this assignment handles routine medication-related activities in accordance with local, Veterans Integrated Service Network (VISN), and national policies and regulations. GS-12 Clinical Pharmacist (Full Performance Level) (a) Experience or Education. In addition to the basic requirements, candidates must meet one of the following: 1. 1 year of experience equivalent to the next lower grade level, or 2. Completion of an ACPE-accredited Pharm.D. program. And Pharmacists assigned to this GS-12 position must demonstrate the following knowledge, skills and abilities (KSAs): 1. Knowledge of professional pharmacy practice. 2. Ability to communicate orally and in writing to both patients and health care staff. 3. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. 4. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: Outpatient pharmacy/retail experience; ASHP accredited residency completion; Board Certification. References: VA Handbook 5005/55, Appendix G15. Physical Requirements: Pre-employment physical is required. The work requires some regular and recurring physical exertion, involving standing or sitting for prolonged periods of time, typing, walking distances within the medical center, lifting/carrying (up to 40 pounds) of pharmaceutical supplies, and bending/stooping/stretching. Mental and emotional stability, working closely with others, normal ranges of hearing and vision (corrected, or with use of aids) ability to effectively communicate, and the ability to distinguish between basic colors and shades of color are required.
