The Chalmers P. Wylie Ambulatory Care Center is recruiting for a Social Worker - Program Coordinator for the Palliative Care Program within the Geriatrics and Extended Care Service.

VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU The duties of the position include, but are not limited to: Responsible for planning, coordinating, budgeting, monitoring, and evaluating care for Veterans. Coordinates the Palliative Care Clinic, taking responsibility for associated performance measures, including access and missed opportunities. Addresses comfort care and end of life issues with veterans and makes appropriate referrals to community agencies. Provides clinical psychosocial and case management services at an advanced practice level to eligible veterans and their family members to meet biological, psychological, social and environmental needs at the end of life. Receives consults, develops and maintains screening methods for evaluation and then delegates needed follow-up for scheduling. Responsible for ongoing development of the Palliative Care Clinic and planning for future growth, including direction for expansion and budgeting. Works with the Service Chief on staff coverage in the event of absence of a Palliative Care team member, as well as assisting in selecting qualified personnel. Responsible for orienting staff facility wide to the Palliative Care Clinic and arranging continuing education. Monitors and evaluates the program with a particular focus on quality and effectiveness, and initiates modification to the program that will promote efficient practice and ensure compliance with VA Ambulatory Care Center and Central Office Directives. Develops and maintains working relationships with community based agencies who serve veterans. Provides direct practice for patients with high risk, complex problems who are terminally ill, and coordinates psychosocial services for those being served by community agencies and the VA. Participates in team meetings and treatment planning with VA personnel and community providers to clarify staff responsibilities, provide assessments and recommendations, and assist in defining care objectives. Offers consultation to colleagues and students on the psychosocial treatment of complicated palliative care patients, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills. Maintains effective therapeutic relationships with patients and families Independently works with patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling and therapy skills. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.. English Language Proficiency. Social workers must be proficient in spoken and written English in accordance with VA Handbook 5005, Part II, chapter 3, section A, paragraph 3j. Licensure. Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). GS-12 Social Worker Requirements Experience/Education. The candidate must have at least two (2) years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one (1) year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Licensure/Certification. Senior social workers must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities (a) Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. (b) Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. (c) Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. (d) Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. (e) Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. References: VA Handbook 5005 Social Worker, GS-0185 Series, Part II, Appendix G39 Social Worker Qualification Standards. Physical Requirements: The position requires a valid government driver's license, knowledge of highway laws/safety, and familiarity with the geographic areas of jurisdiction. The incumbent must be able to operate a motor vehicle and perform local travel as needed. The incumbent may be asked to perform physical tasks requiring standing, bending, stooping, kneeling, climbing stairs, and lifting up to 25 lbs.
