This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Duties

This position is in Pharmacy Program at James J. Peters VA Medical Center. The incumbent serves as program manager responsible for compliance with the standards of the United States Pharmacopeia Chapters 797 and 800 (USP and USP ) on "Pharmaceutical Compounding - Sterile Preparations" and "Hazardous Drugs - The Joint Commission Standards, FDA requirements and VHA policies related to CSPs as outline by VHA directive 1108.12. The Program Manager for Compounding & Hazardous Preparations manages all aspects of compounded sterile, non-sterile and hazardous drug (HD) operations for the VA Medical Center. The Program Manager is responsible for the development, organization and management of the Sterile and Non-Sterile Compounding program in compliance with all applicable laws and regulations (e.g. Joint Commission, USP 797, 795, 800, OIG, FDA, DEA, NIOSH etc. and any future regulations in this area). This includes compliance oversight of all pharmacists, pharmacy technicians, trainees, and other supportive personnel involved in sterile and non-sterile compounding and/or hazardous drug management. The Program Manager is responsible for maintaining reports of testing/sampling performed in our pharmacy areas and acting on the results. Major duties: ADMINISTRATIVE & PROGRAM MANAGEMENT 1. Develops and implements appropriate procedures, oversees and assures facility compliance with USP Chapters 795, 797, and 800 and other applicable laws, regulations, and standards, ensuring competency of personnel and assuring environmental control of all compounding areas. Ensuring the pharmacy service has written procedures for compliance with the USP and USP standards. Reviews sterile and non-sterile compounding policies and procedures at minimum every 2 years or more frequently as needed and recommends updates. Communicates updates and provides training as needed to affected employees. 2. Directs and oversees activities of all assigned operational, technical and professional personnel, with an emphasis on assuring that all USP Chapter 795, 797 and 800 CSP compliance requirements are met in Compounding and Hazardous Drug Management due to their inherent high-risk status about the care of our patients. Maintenance of all required processes and documentation for completely meeting USP Chapters 795, 797, and 800 are a primary focus of this position. 3. Assists in data collection for MUE and participates in quality assurance. 4. Acts as a liaison between Pharmacy Service and other health care providers. Collaborating with Biomedical/Engineering/Facilities Management Services to ensure that Primary engineering Controls (PEC) and Secondary Engineering Controls (SEC) are certified in accordance with USP and USP standards. 5. Coordinates quality assurance monitoring and evaluation involved with compounded sterile preparation and hazardous drug management operations. 6. Ensures the competency of Pharmacy personnel in the preparation of and handling of CSPs through initial, competency assessments, annual training and standard testing within established timeframes. 7. Maintains oversight of the compounding areas and cleanroom primary and secondary engineering controls and ensures they meet environmental standards and validation requirements for viable and non-viable airborne sampling and testing. MEDICATION DISPENSING AND DISTRIBUTION Performs all pharmacy distributive functions according to the VHA and local policies to provide comprehensive pharmaceutical services in a timely fashion to meet the needs of our patients and medical center staff. EDUCATION AND TRAINING 1. Creates an environment to stimulate, encourage and support the participation of pharmacy personnel in providing drug information to the hospital staff and other facilities as deemed appropriate. 2. Completes all mandatory training as required by local, VISN and national mandates within established timeframes and submits documentation of training to the Pharmacy Service Administrative Assistant. Assures timely completion of all internally assigned employee education modules as presented via Talent Management Service (TMS). 3. Attends and actively participates in clinical pharmacy activities, i.e. staff development, clinical pharmacy and pharmacy leadership meetings, residency advisory meetings, and other pertinent activities. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. Physical Requirements. See VA Directive and Handbook 5019. English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: 1) Knowledge of Professional Pharmacy Practices. To be creditable, the experience must have demonstrated the use of knowledge, skills, and abilities associated with professional pharmacy practice. Professional practice means paid/non-paid employment as a professional or unlicensed graduate pharmacist as defined by the appropriate licensing board. (2) Residency and Fellowship Training. Residency and fellowship training programs in a specialized area of clinical pharmacy practice may be substituted for creditable experience on a year-for-year basis. The pharmacy residency program must be accredited by the American Society of Health-System Pharmacists (ASHP). A fellowship program that is not accredited by the American College of Clinical Pharmacy (ACCP) will need to have comparable standards for experience to be creditable (Professional Standards Board refers to the Deputy Chief Consultant for Professional Practice for the determination). (3) Quality of Experience. Qualifying experience must be at a level comparable to pharmacy experience at the next lower level. Experience as a Graduate Pharmacist is creditable provided the candidate was used as a professional pharmacist (under supervision) and subsequently passed the appropriate licensure examination. (4) Part-time Experience. Part-time experience as a professional pharmacist is credited according to its relationship to the full-time workweek. For example, a pharmacist employed 20 hours a week, or on a 1/2-time basis, would receive 1 full-time workweek of credit for each 2 weeks of service. Grade Determinations: In addition to the basic requirements for employment above, the following criteria must be met when determining the grade of candidates. GS-12 Clinical Pharmacist (information for requirements experience for GS 13)Experience or Education. In addition to the basic requirements, candidates must meet one of the following:1. 1 year of experience equivalent to the next lower grade level, or 2. Completion of an ACPE-accredited Pharm.D. program. GS-13 Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Assignments. Candidates at this grade level are to be in one of the assignments listed below. For all assignments above the full performance level, the higher-level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time. Facility Program Manager. Manages all aspects of a distinct program to include policy development, quality resources and specialty functions unique to the program to optimize cost effectiveness (budget) and patient outcomes. Coordinates a single program area of a complex pharmacy operation, or multiple program areas within a facility to develop, organize, manage, and control complex pharmacy programs. Develops, organizes, coordinates and manages single or multiple program areas within the medical facility; this may include a single program area at multiple facilities. The Program Manager title is reserved for those individuals who have one or more of the following: supervisory responsibilities for various sections within Pharmacy Service; a complex program requiring coordination of multiple locations such as medical center care facilities, Community Based Outpatient Clinics (CBOCs), rural health, telemedicine, etc; specialized areas of a complex nature such as nuclear pharmacy, quality assurance, pharmacy informatics, clinical applications coordinator, Home Based Primary Care, pharmacoeconomist or liaison pharmacist duties requiring the coordination of processes with other medical facilities or VISNs. Pharmacists assigned to this position must demonstrate the KSAs. Pharmacists assigned to this position must demonstrate the following KSAs: a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions b. Expert understanding of regulatory and quality standards for their program area. c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters Preferred Experience: PGY1 and/or PGY2, IV Sterile Products experience and knowledgeable of USP 797, USP 800 and USP 795. References: VA Handbook 5005/55, Part II, Appendix G15, dated 06/07/2012. The full performance level of this vacancy is GS-13, which is the actual grade at which an applicant may be selected. Physical Requirements: Some of the work is sedentary, but also demands standing, walking, bending, twisting, stooping, kneeling, pushing, repetitive use of hands and fingers, and carrying/lifting loads of up to 40 lbs. Work demands the ability to perform tasks requiring rapid mental and muscular coordination functions simultaneously if necessary. Must by physically capable of operating various types of equipment used for the job. Potential for musculoskeletal fatigue due to prolonged standing, sitting or other activities. Work Environment: Work is performed in patient care setting and within an outpatient/inpatient Pharmacy setting.
