Learn more Permanent Change of Station (PCS): Not Authorized Financial Disclosure Report: Not required The incumbent is assigned to the Home Based Primary Care (HBPC) program and also serves as the Behavioral Recovery Outreach (BRO) Social Work Coordinator at Sierra Nevada Health Care System under Geriatrics and Extended Care. The primary assignment will focus on HBPC veterans providing high quality case management, psychosocial assessments, advocacy, and to coordinate linkage with appropriate VA and community service providers/agencies as needed by the client. Provides clinical community-based care coordination and case management services to Veterans with complex medical and psychosocial needs, severe functional impairment, and high inpatient and emergency department utilization, in coordination with existing community and VA services. Duties of the position include, but are not limited to: Responsible for all patients admitted to the BRO Program. Manages the daily operation of the BRO program, develops policies and procedures for program operation and prepares reports and statistics for facility, VISN, and national use. Manages and controls the use of data related to the program regarding day-to-day functioning and program evaluation. Ensures the BRO Program is following VHA directives, and that care is rendered pursuant to the guidance of Clinical Practice Guidelines. Acts as a point of contact and liaison for Veterans who are receiving care through the BRO program and will address concerns or issues related to their placement, adjustment, and/or care coordination needs. Completes appropriate documentation, review, and follow-up on Veteran or agency complaints regarding BRO program, enrolled Veterans, and recent discharges from BRO program. Assist with facilitating action for community placements and resources for CLC discharges through collaboration with the veteran and their families, guardians/decision makers as well as interdisciplinary treatment team members to ensure that appropriate community placements are secured in a timely manner. Provide support to the veteran, family, guardian and/or decision maker and the Interdisciplinary Treatment Team. BRO services will be provided for veterans who are diagnosed with a cognitive impairment or are at high risk of developing a cognitive impairment, and those who may otherwise require specialized services to prevent the need for guardianship and/or conservatorship. Complete a comprehensive psychosocial assessment and develop a treatment plan for the veteran in collaboration with the interdisciplinary HBPC Treatment team and Behavioral Recovery Outreach team, the veteran, family members and/or decision maker. The goal of the assessment is to highlight the veteran's strengths, limitations, and internal/external supports and service needs to optimize the veteran's functional status and safely maintain placement in the community. The HBPC assessment will include a home visit. Responsible for the coordination of the referrals of institutional placements in Adult Homes, Assisted Living Programs, or CNH. Educate the veteran, their families, and the team of all the options available to them and will collaborate with the veteran and family on the preferred option. Provide family/caregiver education about VA benefits/resources and county resources, contract nursing home program, respite care, Caregiver Support program, Adult Day Health Centers, Medical services. Attempts to reduce the stresses of hospitalization for veterans and their families. Responsible for developing the treatment plan and setting achievable treatment goals with the veteran/family/decision maker in collaboration with the HBPC interdisciplinary treatment team members and Behavioral Recovery Outreach team. Attend HBPC interdisciplinary care planning meetings and BRO Team care planning meetings. Implement a plan of care addressing specific goals, plans and interventions to decrease stressors that negatively affect the patient and caregiver. Responsible for educating the veteran and/or caregiver of the available services and assisting them in establishing the appropriate referrals based on the veteran's preference or that of his surrogate decision-maker. Experienced in making rapid assessments and developing crisis management plans to maintain patient in the home, for admission to acute, short term and/or long-term placements. Willing to modify tour of duty as necessary to accommodate the needs of Veteran patients. Must have and maintain a valid unrestricted driver's license. Other duties as assigned by management.

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. ? 7403(f). GRADE DETERMINATIONS. In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Social Worker (Program Coordinator), GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts.] References: VA Handbook 5005/120 Part II Appendix G39, Dated September 10, 2019. The full performance level of this vacancy is GS-12. Physical Requirements: Work is primarily sedentary. May require prolonged standing, moderate walking, and manual dexterity. Requires carrying light items such as books, files, or laptop computer. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
