This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws Duties include but are not limited to the following description: Pharmacy Program Manager-Anticoagulation Clinic, GS-13: The Program Manager, Anticoagulation Clinic functions at the highest level of clinical practice and performs duties of a clinical, teaching, and supervisory nature routinely functioning with a high level of autonomy and independent decision-making as an Advanced Practice Provider (APP) operating under a scope of practice. The Program Manager is responsible for managing comprehensive patient care and daily activities of the Anticoagulation Clinic as well as serving as the direct line supervisor of all staff working in the Anticoagulation Clinic. As the direct line supervisor, the Program Manager provides supervision and leadership to all Anticoagulation Clinic staff as appropriate. Under the direction of the Associate Service Chief of Ambulatory Care Clinical Services and the Chief of Pharmacy Services, the Program Manager evaluates clinical skills of pharmacists annually as well as plans and coordinates ongoing training of pharmacists to assure appropriate outpatient clinical competencies are maintained, as required by Joint Commission and other accrediting bodies. Additionally, the Program Manager functioning at the highest level of clinical practice performs services such as patient monitoring, patient counseling, medication management and the prescribing, initiation, modification, and discontinuation of medications within a designated scope of practice. The Program Manager will assist with clinical education activities, serve as preceptor to pharmacy students and residents, provide drug information, contribute to Medication Use Evaluation (MUE) activities, provide support to the Medication Management Committee (MMC) and other hospital committees when called upon, and provide drug information and education to patients. The Program Manager reviews and evaluate appropriateness of non-formulary drug requests submitted by assigned services. As part of the Anticoagulation Clinical duties, the Program Manager has the responsibility for managing patients assigned to the Anticoagulation Clinic for the purpose of preventing stroke, recurrent DVTs, PEs etc. The Program Manager performs necessary services such as physician consultation, patient monitoring, patient counseling and medication management within this clinic. The Program Manager serves as VA Connecticut Healthcare System's Pharmacy representative and special consultant for Anticoagulation Clinic at either campus. Today the sole degree is Doctor of Pharmacy.)(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT) Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. Grade Determinations: Experience: In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. The Program Manager manages all aspects of a distinct program to include policy development, quality resources and specialty functions unique to the program to optimize cost effectiveness (budget) and patient outcomes. Coordinates a single program area of a complex pharmacy operation, or multiple program areas within a facility to develop, organize, manage, and control complex pharmacy programs. Develops, organizes, coordinates and manages single or multiple program areas within the medical facility; this may include a single program area at multiple facilities. The Program Manager title is reserved for those individuals who have one or more of the following: supervisory responsibilities for various sections within Pharmacy Service; a complex program requiring coordination of multiple locations such as medical center care facilities, Community Based Outpatient Clinics(CBOCs), rural health, telemedicine, etc; specialized areas of a complex nature such as nuclear pharmacy, quality assurance, pharmacy informatics, clinical applications coordinator, Home Based Primary Care, pharmacoeconomist or liaison pharmacist duties requiring the coordination of processes with other medical facilities or VISNs. Pharmacists assigned to this position must demonstrate the KSAs identified below: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Functions include but are not limited to the following descriptions: Clinical Patient Care Skills: Utilizes the highest level of clinical privileges as authorized by his/her scope of practice under the appropriate supervision of a physician to design a therapeutic regimen or plan, based on patient-specific information, which includes establishing a therapeutic endpoint, prescribing an appropriate drug therapy and ordering appropriate laboratory monitoring for effective disease state management. Interfaces with providers (including physicians, PAs, APRNs), as well as other services (including Nursing, Social Work, Mental Health, etc.) in the design of a patient-centered treatment regimen to assure that medication therapy is both appropriate and cost-effective. Independently manages unstable pharmacotherapy patients in his/her pharmacotherapy clinics, improving system efficiency and quality. Provides care/counseling for patients ranging in age from 18 through the geriatric years taking into consideration knowledge-based competencies which include diagnosis, physical, psychological, cultural, moral, health literacy and religious needs of patients. Employs tailored counseling strategies to maximize effective communication with patients, such as patient-friendly language in verbal and/or written form, teach-back methods, and motivational interviewing. Counsels patients on proper self-management for their chronic disease states, including optimization of medication adherence, therapeutic lifestyle interventions, and self-monitoring as appropriate (e.g., home blood pressure, height, weight, etc.) Medication Dispensing and Distribution: Performs all pharmacy distributive functions according to the VHA and local policies to provide comprehensive pharmaceutical services in a timely fashion to meet the needs of our patients and medical center staff. Exhibits a high degree of accuracy in receiving and processing of prescriptions (manual and electronic). Exhibits ability to be highly productive in a professional environment. Administrative/Supervisory/Program Management: Participates in the development and enforcement of Pharmacy and Health System policies and procedures. Assures that the VA Connecticut anticoagulation program meets the requirements of the National VA Anticoagulation Directive. Serves as the lead subject matter expert (SME) in the oversight, design, implementation, and function of the anticoagulation management program. Collaborates on the development of processes and procedures in areas where anticoagulants are used with practice area leads. Standardizes practice for the anticoagulation management program through activities such as educational initiatives, Preferred Experience: Must possess an advanced set of knowledge and skills in managing and educating patients receiving medication therapies as evidenced by completion of a pharmacy practice residency and/or board certification or equivalent experience. The clinical pharmacy specialist is encouraged to become board certified or complete a certification within three years of being in the position. (i.e. CACP, BCPS, BC-ACP, CGP). Completion of PGY-1 Pharmacy Residency and/or at least three years of direct patient care experience as a Clinical Pharmacy Specialist with a scope of practice. Experience working in a VA Anticoagulation Clinic or managing patients on Anticoagulation Therapy. Supervisory experience. References: VA Handbook 5005, Part II, Appendix G15, Licensed Pharmacist Qualification Standard) dated June 7, 2012. Physical Requirements: The employee must be able to talk, listen, and speak clearly on the phone. Must be able to use a computer with both hands. Must be able to sit for prolonged periods of time as well as stand. Must be able to lift up to 25 pounds.
