This position is located within the Whole Health Coach program at the Lexington VA Medical Center- in Lexington, KY. The Social Science Program Coordinator - Whole Health Coach is responsible for the delivery of Whole Health Coaching and skill development of the local Medical Center's Whole Health Coach program, including provision of care through coaching and promotion and education regarding Whole Health Coaching to Veterans and staff.
Duties

Duties may include, but are not limited to the following:
Provides health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones;
Conducts interventions for Veterans using one-on-one, couple, or group modalities in outpatient and inpatient settings;
Provides eligible Veterans and family members with information regarding lifestyle management including weight management, nutrition, exercise/fitness, smoking cessation, stress management, diabetes, pain management, mental health, etc.;
Provides follow-up care through telephone clinics or secure messaging between face-to-face appointments;
Reviews current health behaviors and develops customized treatment plans with Veterans;
Schedules ongoing sessions to assess progress and assist in navigating obstacles or challenges;
Coordinates care with Veterans' treatment providers, both primary and specialty care;
Conducts outreach activities to inform Veterans and family members of available programs;
Develops and distributes locally approved health education materials to identified enrolled Veterans and family members;
Participates in leadership roles on local, state and/or national professional organizations specific to health coaching;
Assists with data management analyses, special studies, and reports for use by management for budget preparation, workload projections, and quality assurance and utilization reviews;
Participates in the establishment of administrative standards locally; the objective of which is intended to improve programs, facilities and operations; and,
Performs all other duties as assigned.
Work Schedule: Monday - Friday, 8:00am-4:30pm

Qualifications
ENGLISH LANGUAGE PROFICIENCY: In accordance with 38 U.S.C 7402(d), No person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Are you proficient in basic written and spoken English
AND
BASIC REQUIREMENT:
Four (4) years of appropriate experience that demonstrates you have acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field of Social Science Program Coordinator. (This experience must be supported by your resume to be creditable.)
OR
A degree in behavioral or social science; or related disciplines appropriate to this position of Social Science Program Coordinator. (A copy of your transcript must be provided at the time of application).
OR
A combination of education and experience that provided you with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field of Social Science Program Coordinator.
MINIMUM QUALIFICATION REQUIREMENTS GS-9
Specialized Experience: At least one (1) full year of specialized experience equivalent to at least the next lower grade level (GS-7) in the Federal Service, that is directly related to the work of the position and has equipped you with the particular knowledge, skills and abilities to successfully perform the duties as a Social Science Program Coordinator. Qualifying specialized experience includes: reviewing supporting material to determine compliance with regulations and policy requirements; assists in planning and implementing program(s) within a medical center setting; using creativity to engage employees and community partners in a newly developed program; developing training materials for staff educational needs; and experience using a wide range of qualitative and quantitative methods for the assessment and improvement of program effectiveness.
OR
Education: Successfully completed two (2) years of progressively higher-level graduate education leading to a master's degree or master's or equivalent graduate degree in a behavioral or social science; or related disciplines. Education at the graduate level must have been obtained in an accredited college or university and must demonstrate the knowledge, skills, and abilities necessary to do the work of this position. OR
Combination: Combinations of successfully completed graduate level education (beyond the first year) and specialized experience as described above, to meet the total experience requirements. The education portion must include graduate courses that demonstrate the knowledge, skills, and abilities necessary to do to the work of this position. This education must have been obtained in an accredited college or university. NOTE: Transcripts must be submitted with your application materials. Education cannot be credited without documentation. Experience must be clearly indicated in your resume.
