The Program Support Assistant provides specialized and expert administrative patient support while working collaboratively in an interdisciplinary coordinated care delivery model. Work involves specialized administrative judgment and the flexible use of a wide range of clinical flow processes relating to access to care across multiple clinics, specialties, and/or care in the community resources. This position functions as an integral part of the VISN Community Care support team.

Duties

Major duties include but are not limited to: Providing specialized and expert administrative patient support while working collaboratively in an interdisciplinary coordinated care delivery model. Developing and maintaining effective and efficient communication with the patient, interdisciplinary coordinated care delivery model teams, VA medical centers, and other agencies. Using a wide range of clinical flow processes relating to access of care across multiple clinics, specialties and/or care in the community resources. Setting priorities and deadlines while adjusting the flow and sequencing of the work to meet team and patient needs. Entering appropriate information into the patients electronic record. Monitoring pre-appointment information and requirements to assure readiness for patient visit/procedure. Managing patient systems to verify and validate accuracy and resolve issues. Evaluating patient information and clinic schedule lists to determine whether the patient requires an immediate appointment. Participating in team huddles and team meetings to manage, plan, problem solve and follow up with patient care by sharing information and collaborating with the interdisciplinary team. Assisting the team to reinforce the plan of care and self-help solutions. Informing team members about shared patients who receive their care at multiple VA centers or those who receive care in the community under multiple specialty areas. Ability to demonstrate and execute highly developed skills with MS Excel and multiple health care operating systems. Work Schedule: Full-Time Monday - Friday, 8:30am to 4:30pm Position Description Title/PD#: Program Support Assistant/99747-S Physical Requirements: The work is administrative in nature, primarily sedentary. Occasionally light physical effort may be required. As most of the time is spend using a computer, actions must be taken to prevent wrist, eye, and back injuries.

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 08/07/2023. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-06 position you must have served 52 weeks at the GS-05. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. You may qualify based on your experience and/or education as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade (GS-05) in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: knowledge of health care delivery systems related to financial management, management of physical and human resources, project management, process improvement methods, statistical analysis, data generation and evaluation in order to perform the day to day work; knowledge of standards and regulations from external accreditations bodies, including The Joint Commission, Council on the Accreditation of Rehabilitation Facilities, Occupational Safety and Health Administration, Long-Term Care Institute, Utilization Review Accreditation Commission incorporated (URAC), International Customer Management Institute (ICMI) and others; knowledge of and be technically proficient in the application of numerous laws, VA regulations, and local policies and procedures, as well as other directives relating to the multiplicity of medical and administrative fields covered by the assigned duties; knowledge of the overall VHA organization, local facilities, organizational VISN structure, and facility services is necessary to perform the essential functions of the position. Ability to integrate ethics, privacy, and confidentiality measures into decisions and program and to establish and maintain effective work relationships with a variety of individuals, groups, and programs. Ability to professionally communicate verbally and in writing with diverse internal and external customers.*Education is usually not credible above the GS-5 for this position; however, graduate education may be credited in those few instances where the graduate education is directly related to the work of the position. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment. Physical Requirements: The work is mostly sedentary, there can be demands for standing, walking, bending and long periods of sitting at the computer, answering the telephone, and transferring calls frequently.
