The New Mexico VA seeks a Nurse Case Manager (NCM) to coordinate care for patients in the Spinal Cord Injury and Disorders Service (SCI/D).

Duties

The SCI/D NCM is responsible for case management beginning from the time of referral to the Hub and through the patient's discharge from the SCI/D inpatient unit. The NCM is an active member of the interdisciplinary team (IDT) and collaborates with team members in planning care for patients with complex care needs. The NCM is proficient in utilizing the nursing process to assess, plan, implement, evaluate, and document patient care. In collaboration with SCI/D Interdisciplinary team, the NCM coordinates the discharge process and ensures treatment recommendations are provided to the patient and/or the facility providing the patient's next level of care, based on physical, psychological, educational, safety, support systems and related criteria appropriate for the veteran. With administrative oversight by the Nurse Manager of the service, the Nurse Case Manager is a registered professional nurse, qualified by education and experience to provide direct staff support in planning, designing, integrating, implementing, modifying, and evaluating the effectiveness of program components and/or services to improve the quality of patient care. SCI/D Nurse Case Manager Functions: Responsible for coordinating provisions of care for SCI/D patients to ensure quality and effectiveness of care CARF leader in accreditation and completion of CARF disclosure for patients Lead of CARF meetings with interdisciplinary team to maintain accreditation standards and auditing Collaborates with team completing CARF disclosure form with acute injuries Participates in CARF data collection, analysis, and dissemination, remaining focused on outcomes to ensure success Coordinates supplies for patients and manages equipment procurement to ensure arrival prior to discharge Coordinates home modifications to ensure timely completion for safe discharge Assists with scheduling and appointments with community providers Plans care for New Spinal Cord Injury Veterans across our catchment of New Mexico and Texas, tracking SCI/D in patients admitted to other hospitals or other floor within our facility, recruiting patients eligible to be admitted to Albuquerque SCI/D Hub Liaising with referring care teams in the community and acquiring clinical documents necessary for screening as well as assisting the SCI Physician with review of these documents. Spokesperson for National Hub and Spokes meetings across National SCI/D centers Collaborates with social work to facilitate discharge and team meetings for patients, etc. Coordinates with SCI/D Interdisciplinary team for skilled community health consults related to discharge planning Participates in SCI/D Task Force for increase in annual evaluation completions Participates in SCI/D weekly interdisciplinary team meetings Coordinates and ensures clinical care and assessment of SCI/D patients that are off service with interdisciplinary team Coordinates home evaluations of SCI/D inpatients prior to discharge with SCI/D Interdisciplinary Team Assists outpatient clinic with services/consults needed such as prosthetics, community home health, equipment, etc. Able to assess, plan, implement, monitor, and evaluate options and services to affect an appropriate, individualized plan for the veteran across the continuum of care. Utilizes clinical knowledge and competence, positive communication skills, problem solving and conflict resolution techniques. Demonstrate strong skills in assessment, organization, and time management. Able to consult with the appropriate services to meet the veteran's needs. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Bachelor's Degree of Science in Nursing (BSN) or higher 1 year SCI/rehabilitation experience 1 year Nurse Case Manager experience Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Moderate lifting (up to 44 lbs); moderate carrying (up to 44 lbs); use of fingers; both hands required; walking, climbing; both legs required; operation of motor vehicle; both eyes required; hearing aid permitted; sitting at a computer.
