The HUD-VASH Clinical Assessor - Physical Therapist (PT) will be referred to herein as "PT Clinical Assessor" or "Incumbent". The primary function of this PT Clinical Assessor is direct patient care, including completing functional assessments to a diverse population in outpatient and community settings within the Northern California HUD-VASH Program.The incumbent will complete functional assessments of Homeless Veterans utilizing a prescribed instrument.

VA Careers - Physical Therapy: https://youtube.com/embed/Gi2hoFqIoqY - Accept and respond to PT/HUD-VASH consults and document evaluation and treatment in compliance with HUD-VASH and Physical Therapy policies. - Complete Veteran functional assessments. - Possesses skill in independently utilizing appropriate screening and standardized and non standardized evaluation tools required to appropriately provide and direct the PT care plan in multiple settings. - Responsible for the independent comprehensive assessment of functional roles and provide direct intervention with varying degrees of complexity to enhance safety, wellness, participation, and performance in all areas of physical functioning of Veterans referred to the HUD-VASH Program. - Demonstrates knowledge of and ability to independently apply clinically appropriate assessment techniques and treatment interventions to address and evaluate all aspects of the physical therapy domain: occupations, client factors, performance skills, performance patterns, contexts, and environments. - Able to utilize standardized and non-standardized assessments methods to communicate information to the Centralized Eligibility & Appeals Teams (CEAT) and interprofessional HUD VASH Team about Veterans' functioning. Communication: - Demonstrates the teaching skills required to provide education to Veterans, family members, caregivers, students, and other healthcare professionals. - Demonstrates the ability to effectively communicate, both orally and in writing, with Veterans, family members, supervisors, caregivers, students, and other healthcare professionals. Effectively utilize telehealth/virtual as well as telephone and in-person visits to provide care to Veterans in the HUD-VASH Program. - Establishes and maintains contact with other health care providers involved in Veteran care via written, telephone, and personal communication on a regular basis. - Participates in interdisciplinary care planning conferences, as assigned. Documentation: - Performs comprehensive written documentation of evaluations/assessments, progress notes, and discharge summaries. - Documentation is consistent with regulatory bodies and accrediting commissions, VHA policy, and facility directives regarding content and timeliness. - Proactively communicates with Prosthetics and Sensory Aide Services (PSAS) to address Veterans' needs for adaptive/durable medical equipment, orthoses, and prosthetics devices, in accordance with facility policy. - Documents Veteran functional assessments for HUD-VASH in a timely manner and communicates information from assessments to the interprofessional CEAT for timely processing of the application. Education: - As assigned, demonstrates the teaching skills as required to provide education to Veterans, family members, caregivers, employees, students, and coworkers. - Educates multiple service lines within the facility about the Physical Therapy scope of practice. - The PT continually seeks to improve knowledge and skills by reviewing current literature, conducting in-service training, participation in presentations, and attendance at continuing education programs, especially in their area of expertise. - Continuing education and training will be focused on maintaining and improving competency in all general clinical functions but will also include coursework directed toward the development and improvement of clinical instruction, data collections and reporting techniques, and utilization and reporting of outcome measures. Clinical: Evaluations: Take place primarily in the home and the community and occasionally in a VA Clinic area, such as a VA OPC, individually or in groups. Conducts an evaluation that gathers information from the medical record, interview of the Veteran and caregiver, family member, or other providers, observation, and standardized and non-standardized testing. Analyzes the demands of the activity. PT may also conduct ongoing wellness contacts via in-home, telephone, or telehealth interviews (VVC) to ensure the needs of the eligible veteran are met over time, and other care needs may be addressed through referrals to other VA and non-VA services, as appropriate, at appropriate intervals determined by the National and/or local HUD-VASH Program. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Individuals appointed under authority of 38 U.S.C. chapters 73 or 74, to serve in a direct patient-care capacity in VHA must be proficient in written and spoken English. See Chapter 2, section D, paragraph 5a, this part. Education and Experience: The individual must meet at least one of the requirements below: Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a physical therapist. Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a physical therapist. Doctorate degree in physical therapy. NOTE: Individuals must be a graduate of a Commission on Accreditation in Physical Therapy Education (CAPTE) accredited college or university. The CAPTE is the only accreditation agency recognized by the United States Department of Education (USDE) and the Council for Higher Education Accreditation (CHEA) to accredit entry-level physical therapy programs. Verification of accredited programs may be obtained from the American Physical Therapy Association (APTA) at www.apta.org. Licensure: Individuals hold a full, current, and unrestricted license to practice physical therapy in a State, Territory or Commonwealth of the United States, or in the District of Columbia. Non-licensed PTs, who otherwise meet the basic requirements in this standard, may be given a temporary appointment as a graduate PT at the GS-11 grade level under the authority of 38 U.S.C. 7405 (a)(1)(D) for a period not-to-exceed two years from the date of employment on the condition that such PT provide care only under the supervision of a PT who is licensed. Failure to obtain licensure during that period is justification for termination of the temporary appointment. Grade Determinations:GS-11 (Entry Level)Education, Experience, and Licensure: None beyond the basic requirements. Non-licensed PTs, who otherwise meet the basic requirements in this standard, may be given a temporary appointment as a graduate PT at the GS-11 grade level under the authority of 38 U.S.C. 7405 (a)(1)(D) for a period not-to-exceed two years from the date of employment on the condition that such PT provide care only under the supervision of a PT who is licensed. Failure to obtain licensure during that period is justification for termination of the temporary appointment. GS-12 (Full Performance Level)Education, Experience and Licensure: In addition to the basic requirements, candidates must possess one year of experience equivalent to the GS-11 grade level, hold a valid unrestricted state license and demonstrate all the KSAs below. Demonstrated Knowledge Skills and Abilities: Ability to make autonomous clinical decisions in a Direct Access environment. This includes ability to independently evaluate and treat patients who seek physical therapy services. Skill in performing examinations and evaluations of individuals who have or may develop impairments, activity limitations, and participation restrictions related to conditions of the musculoskeletal, neuromuscular, cardiovascular, pulmonary, and/or integumentary systems while considering the effects attributable to unique psychosocial and environmental factors. Ability to interpret findings from examination and evaluation, select appropriate test and measures, and integrate findings into the physical therapy plan of care for the full range of patient populations. Ability to establish a diagnosis within the scope of physical therapy and identify the appropriate rehabilitation intervention, including referral to another provider for further consultation as clinically indicated. Ability to determine physical therapy prognosis by incorporating examination findings with the patient's preferences in order to set clinically appropriate treatment goals, optimize outcomes and maximize functional independence. Ability to independently provide clinical oversight of Physical Therapy Assistants as well as students on clinical affiliations who are in Doctoral PT Programs or PTA Programs All qualifying experience you possess must be clearly described in your application package.
