VA Portland Health Care System is a Complexity Level 1a Facility and provides extensive primary, secondary, tertiary, and quaternary care in outpatient, inpatient, residential, and community settings. VAPORHCS is a multi-site medical system that includes a major medical facility in Portland, Oregon, outpatient and residential facilities in Vancouver, Washington, and 10 Community Based Outpatient Clinics.

VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws The duties of this position include but are not limited to the following: The Associate Chief of Pharmacy for Clinical Services & Education must be a licensed pharmacist and is accountable to the Chief, Pharmacy Service as his/her assistant for providing leadership, direction, organization, planning, and implementation of pharmacy programs which promote effective utilization of all resources and promote quality pharmaceutical services. He/She serves as the Acting Service Chief in the absence of the Service Chief. The incumbent develops and maintains a system of internal reviews that ensure service programs operate at a satisfactory level of performance and in compliance with regulatory and accrediting bodies. He/She is responsible for pharmacy utilization of resources and budget and is responsible for supervision, management, and coordination of all clinical pharmacy programs within the Pharmacy Service. Responsibilities include the management and/ or oversight of multiples clinical programs in both the Ambulatory and Acute Care settings including but not limited to the Primary Care PACT program, Mental Health, and Pain Management services, Anticoagulation program, Oncology, Infectious Diseases, Critical Care, Specialty programs and Pharmacy Training programs. Shares the responsibility for ensuring compliance with a wide variety of regulatory requirements. VHA policy must establish a foundation for local medical center memorandums (MCMs). The MCMs will ensure compliance with VHA Directives and Handbooks, Drug Enforcement Agency (DEA), The Joint Commission (TJC) and the Office of the Inspector General (OIG). Compliance with the MCMs will include: TJC standards, medication management, environment of care, and applicable National Patient Safety & National Pharmacy Benefits Management goals that are integral part of pharmacy planning, policies, and procedures. Ensures that the Medical Center and Pharmacy Division policies and procedures related to Clinical Pharmacy are reviewed and/or revised in a timely manner before expiration or as changes occur. Creates new policies when required. Demonstrates knowledge of medical center memorandums, policies and procedures related to pharmaceutical services and thorough knowledge of Pharamcy Service policies, procedures, memorandums, and directives applicable to areas of responsibility. Plans, directs, manages, monitors, evaluates, enhances, and promotes Clinical Pharmacy Services provided by the Pharmacy Division. Oversees the effective utilization of resources and budget to support the mission and goals of the organization. Utilizes staffing standards, creates staffing models and assesses workload capture information meet productivity and provide safe, efficient care. Establishes section goals and objectives consistent with the department and medical center goals and develops operations plans and polices to achieve them. Assist the Chief and other pharmacy leadership in establishing and accomplishing pharmacy's missions and goals. Establishes a system of planning daily activities of Clinical Pharmacy services with short and long- term strategic goals consistent with the strategic priorities of the pharmacy division and integrated with the medical center strategic priorities and goals. Promotes a patient centered, team- based multidisciplinary model of pharmaceutical care to promote optimal patient health related outcomes. Promotes a patient focused team- based model of pharmaceutical care to support medication prescribing and monitoring that assures positive outcomes for medication use. Provides high quality, age specific pharmaceutical care to all veteran patients. Supports the development of a "continuum of care" model for veteran patients. Promotes the rational and safe use of medication by providing unbiased drug information, drug monitoring, usage evaluations services, and drug consultations. Identifies continuing education needs based on performance of all subordinate staff. Preferred Experience-PGY 1 and PGY 2 Pharmacy Residency; Supervisory, management and leadership experience. Greater than 10 years of experience as a clinical pharmacist specialist with scope of practice to prescribe medications. Developing and maintaining a system of internal reviews that ensure service programs operate at a satisfactory level of performance and in compliance with regulatory and accrediting bodies supervision, management, and coordination of clinical pharmacy programs management and/ or oversight of multiples programs simultaneously. Education (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations:GS-14 (a) Experience. In addition to the GS-13 requirements, must have 1 year of experience equivalent to the next lower grade level. Assignments. Candidates at this grade level are to be in one of the assignments listed below. . For all assignments above the full performance level, the higher level duties must consist of significant scope, administrative independence, complexity (difficulty) and range of variety as described in this standard at the specified grade level and be performed by the incumbent at least 25% of the time.
