VA Portland Health Care System (VAPORHCS) is a Level 1A interdisciplinary teaching and quaternary facility with service-lines serving primary care, emergency medicine, medicine, surgery, specialty, psychiatry, physical medicine and rehabilitation, neurology, oncology, dentistry, geriatrics and multiple community-based outpatient clinics (CBOC).

Duties

The Quality, Safety, and Value (QSV) - Patient Safety & Risk Awareness Nurse Manager is a full-time professional nurse in the service, reporting to the Director of QSV. The Patient Safety & Risk Awareness Nurse Manager will serve as an organization content expert and provide oversight of VAPORHCS Patient Safety & Risk Management program throughout the organization to include affiliated community-based outpatient clinics (CBOCs). Duties include, but not limited to: Administration of the Integrated Risk Management Program Ensuring compliance with VHA and Network Directives, as well as laws and regulatory standards pertaining to the Integrated Risk Management program, including those laws and regulatory standards applicable to the healthcare community outside of the purview of the Veterans Administration. Consulting for executive leadership team (ELT) and VAPORHCS employees on risk management/patient safety related issues Providing comprehensive reporting and analysis of all programmatic outcomes and make recommendations to top management. Communicating and collaborating with non-Department of Veterans Affairs (VA) facilities as well as VA health care professionals and administrators. Maintaining knowledge of current VA Guidelines, policies, Federal regulations, and laws. Providing strong leadership, demonstrate expertise in the role of the administrator and program manager, and be comfortable in the role of educator, and direct service provider. These responsibilities will be dictated by the needs of the program and may change at the program progresses. Maintaining continuous accreditation readiness of his/her units Responsible for performance improvement data, customer service/patient satisfaction, patient and employee safety, interviewing applicants, employee relations, standards of care and practice, the work environment, and accreditation and regulatory standards develops and manages collaborative partnerships with internal and external customers Staff scheduling and coaching Dealing with employee conduct and performance issues, Completing recurring reports and performance evaluations in a timely manner. Participation in service and health care system committees and task forces to improve nursing practice and patient care. Identifying educational needs of staff and ensures that these needs are addressed. Works in partnership with the union and adheres to EEO principles. The Patient Safety & Risk Awareness Nurse Manager is responsible for program oversight, direction, coordination and leadership for the Risk Management Program and the Patient Safety Program, specific leadership responsibilities included: Coordinates Integrated Risk Management Program throughout VA Portland Health Care System. Monitors, reports, and analyzes risk management data. Collaborates with all VA Portland Health Care System programs/activities. Ensures that identified reportable events are reported to appropriate governing agencies as outlined in Directives, National Center for Patient Safety (NCPS) within the specified time frames as established per guidelines/directives. Ensures the completion of investigations, action items, and/or reports to the governing agencies. Ensures the completion of the Administrative Investigation Board (AIB) is completed in a timely manner. Promotes a Culture of Safety, Just Culture, and High Reliability. Ensures all mandatory Patient Safety reports are reported timely to the NCPS. Ensures the timeliness of Root Cause Analysis (RCA), Healthcare Failure Mode and Effect Analysis (HFMEA), and other Patient Safety Activities are implemented and completed. Responsible for direct supervision of Patient Safety, Risk Management, Patient Safety Nurses, and Risk Management Nurses. Responsible for guidance and direction to Executive Leadership regarding external review readiness (the Joint Commission, Office of General Inspector, and other accrediting agencies) and for the performance of quality indicators. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Thorough knowledge of patient safety policies and activities. 1-2 years' experience with root cause analysis, HFMEAs, Joint Patient safety reporting, ability to train and implement programs, certified professional in Healthcare Quality and Lean/ Six Sigma. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.
